    Malaria hypnozoites resistant to chloroquine

Previously referred to as “Resochin”, Chloroquine was first discovered in 1924 at Bayer laboratories. Its first use was initially ignored as it was found to be toxic to people.

Drug-resistant P. falciparum. Chloroquine-resistant P. falciparum first developed independently in three to four areas in Southeast Asia, Oceania, and South America in the late 1950s and early 1960s. Since then, chloroquine resistance has spread to nearly all areas of the world where falciparum malaria is transmitted. Emerging resistance to chloroquine CQ by Plasmodium vivax threatens the health of the hundreds of millions of people routinely exposed to the risk of infection with this organism. CQ has been the first-line therapy for vivax malaria since 1946 32, 115. Plasmodium falciparum developed resistance to CQ in the 1950s, and today it occurs globally. Plasmodium falciparum chloroquine resistance is a major cause of worldwide increases in malaria mortality and morbidity. Recent laboratory and clinical studies have associated chloroquine resistance with point mutations in the gene pfcrt. However, direct proof of a causal relationship has remained elusive and most models have posited a multigenic basis of resistance.

These trials showed that chloroquine eliminated malaria and was appropriate to be used as an antimalarial drug. Thus, it was not until World War II that the government of the United States sponsored the clinical trials of chloroquine as an antimalarial drug.

The loss of chloroquine to resistance in vivax malaria impels the selection of new companion drugs to primaquine for radical cure of this infection. If the hypnozoitocidal activity of primaquine requires an appropriate companion drug, the efficacy of primaquine against relapse must be reestablished with each new blood schizontocide paired with it. We screened 2236 subjects for malaria infection through active case detection and identified 232 infected cases with 100 subjects carried P. vivax mono infection.

Psoriasis Associated with Plaquenil Hydroxychloroquine - Dr. Skin biopsies of the new rash confirmed the diagnosis of psoriasis. Plaquenil was discontinued. Because his blood contained anti-SS-A antibodies, rendering him highly sensitive to ultraviolet light, he was not a candidate for phototherapy which could flare lupus especially after Plaquenil was stopped.
